De-identified aggregate participant data supporting the findings of this study are available within the article and its Supplementary Information. Individual participant data cannot be shared publicly owing to participant privacy restrictions and the terms of the institutional ethics approvals. Requests for access to de-identified participant-level data will be reviewed by the corresponding author and the sponsor's data access committee, subject to a data use agreement and a defined analysis plan. The statistical analysis code (SAS and R scripts) is available on GitHub at [TO_BE_PROVIDED] under the MIT licence.
